Registration Procedure

1. 01Make an appointment (in-person or by phone)
2. 02Request a “Special Calculation of Health Insurance Registration Request Form” from your attending physician
3. 03Submit the completed form to the special calculation registration officer at the Administration Office → Be sure to accurately write down your address, as the National Health Insurance Service (NHIS) will mail your registration certificate to that address
4. 04Return home (the request form may be submitted electronically via EDI or directly by the patient.)

Eligibility Criteria for Benefits

Patient With a Serious Illness

• When receiving outpatient or inpatient treatment under medical benefits, the patient is responsible for 5% of the total medical benefit cost
• Eligible Patients: Table 3, Standards for Special Calculation of Patient Co-payment

Patients with rare and intractable diseases (Article 5 of the Standards for Special Calculation of Patient Co-payment)

• Eligible Patients: Table 4, Standards for Special Calculation of Patient Co-payment

For other special calculation beneficiaries, please refer to the “Standards for Special Calculation of Patient Co-payment” as announced by the Ministry of Health and Welfare.
