Referral Process

• If you are hospitalized, please consult with your physician and nurse about 3–4 days before discharge, or if you are at home, visit the outpatient clinic to discuss with your attending physician.
• Before discharge, a home nursing nurse will visit your room to meet with you and your family to explain the process. Home nursing services can begin immediately after discharge.
• During your consultation, you may apply for home nursing through your physician and discuss the procedures with a home nursing nurse.
• Home visits are generally scheduled 4–8 times per month, depending on the patient’s condition.
