What is Home-Based Hospice Care?

Home-based hospice care is a service in which a hospice team, including a physician and a specialized nurse, visits the patient’s home—the place preferred by terminal cancer patients and their families—to provide physical, psychological, social, and spiritual care, helping them maintain quality of life and live comfortably.

Eligibility

  • Patients diagnosed with terminal cancer
  • Terminal cancer patients for whom further aggressive chemotherapy are no longer beneficial
  • Patients requiring pain relief and symptom management for cancer-related discomfort
  • Patients referred to hospice and palliative care by their attending physician

Application Process

  • Consultation with the home hospice nurse

  • Apply at the Family Medicine outpatient clinic or upon discharge

  • Complete the consent form

  • Home visit

Service Area

Daejeon, Sejong, Chungcheongnam-do, and Chungcheongbuk-do (within one hour travel distance one way)

Frequency of Visits

The number of visits is determined through consultation between the home hospice team and the family, depending on the patient’s condition.

  • Comprehensive assessment and care planning
  • Pain and symptom management (pain control, IV therapy, wound care, etc.)
  • Tube replacement and pressure sore management
  • Blood sampling and laboratory testing
  • Education and counseling (patient care, symptom control, end-of-life care, home death preparation, etc.)
  • Therapeutic programs (art therapy, music therapy, aromatherapy, etc.)
  • Volunteer support services
  • Connection to inpatient hospice services

    • Mobile IV stand, air mattress, portable commode, wheelchair, oxygen concentrator, suction device, etc.
