This is a guide to the required documents for obtaining copies of medical records or image data (CD).
To protect the privacy of our patients and prevent the infringement of personal rights, this hospital manages medical records in accordance with the Medical Service Act and the Personal Information Protection Act.
When issuing copies of medical records, the applicant’s identity must be verified. Therefore, the patient, a family member, or a legal representative must bring the required documents listed below when visiting the hospital.
According to Article 21, Paragraph 2 of the Medical Service Act, medical records shall not be disclosed, viewed, or copied by anyone other than the patient. However, in accordance with Article 21, Paragraph 3 of the same Act and Article 13-3 of its Enforcement Regulation, copies may be issued if the specified legal requirements are met.

Medical records related to disability assessments or Agent Orange examinations (test results, imaging data) can only be issued upon request to the regional Veterans Affairs Office after the applicant has received the evaluation results.

Identification: Resident Registration Card, Passport, Driver’s License, or other government-issued photo ID verifying identity.
Consent and power of attorney forms must be completed using official Form No. 9-2 or 9-3.
The consent form must clearly state the purpose, date, and scope of consent.
Both the consent and power of attorney forms must include the patient’s handwritten signature. (stamps or fingerprints are not accepted)
For minors under 14 years of age, issuance is allowed only if the applicant is deemed competent (typically age 10 or older) and possesses a valid photo ID (Youth Card or Student ID).
If the patient consents to the appointment of a sub-representative, the family member or representative may be authorized to request copies on the patient’s behalf. The consent or power of attorney must explicitly include the statement “Sub-representative appointment is permitted.”

If a family member entitled to request copies on behalf of the patient appoints a representative, the representative may apply for the copies.
If the patient has no family members (spouse or lineal ascendants/descendants of the patient or spouse), the patient’s siblings may request copies of medical records, and authorized family members may appoint another representative.
If the patient has consented to the appointment of a sub-representative, the authorized family member or representative may delegate the request to another representative. (The consent form must explicitly include the statement “Sub-representative appointment is permitted.”)
