Medical Opinion or Diagnosis Certificate
Primary Diagnosis: Specify the type of cancer and corresponding cancer code
Include a statement indicating that the patient is in the terminal stage and requires hospice care
If the consultation is made by a family member
Bring a Certificate of Family Relationship and an ID Card
Documents to Bring on the Day of Admission
If transferred from another hospital: Recent blood test results and one-week medication record
If coming from home: Current medication prescription
Palliative Care Consent Form
In hospice and palliative care, patients and their families are supported in maintaining quality of life, and a prior consent form is obtained to refrain from unnecessary life-prolonging medical interventions such as special treatments, diagnostic tests, or cardiopulmonary resuscitation.
