Counseling Services

  • Eligible Participants: Any patient or guardian may receive counseling.
  • How to Request Counseling
    • Referral from your attending physician or nurse
    • Direct visit or call the Social Welfare Office by patient or guardian

      Location: West Wing, Basement Level 1 (former injection room)

Counseling Procedure

  • STEP 01

    Counseling Intake and Referral
    • Referral by medical staff or ward (EMR or phone)
    • Direct visit by patient or guardian
    • Referral from community organizations
    • Direct screening by a medical social worker
  • STEP 02

    Initial Interview
    • Collect personal background information
    • Assess the current situation and needs of the patient and guardian
  • STEP 03

    Assessment
    • Psychosocial and emotional issues
    • Issues related to admission, discharge, or rehabilitation
    • Financial difficulties such as medical expenses
    • Crisis situations
  • STEP 04

    Goal Setting
    • Establish goals and priorities for each issue
    • Decide on the methods of intervention
    • Develop an intervention plan
  • STEP 05

    Intervention
    • Provide individualized services according to the intervention plan
    • Connect patients with community resources
  • STEP 06

    Evaluation and Termination
    • Evaluate progress toward intervention goals
    • Terminate or conduct follow-up management based on the evaluation results
    • Refer the patient to a community organization if necessary
