Referral Procedure

  • For Inpatients : Apply for home nursing through the attending physician or nurse before discharge.
  • For Outpatients : Apply after an outpatient consultation in the relevant department.
  • For Patients from Other Hospitals : Bring a referral letter or medical certificate from the other hospital, then consult the appropriate department to apply for home nursing.

Referral Procedure

  • Outpatient Home Nursing Referral
    1. Outpatient appointment (02-2225-1234)
    2. Registration at the administration office
    3. Consultation and physician interview → Home nursing referral
    4. Home nursing consultation: Main Building 5F / Cost + visit area + visit frequency / Referral to partner hospitals or welfare services if needed
    5. Complete consent form
    6. Education on home patient care methods
    7. Purchase and preparation of patient supplies
    8. Deposit payment at administration office * Government-covered patients: No deposit required
    9. Discharge and return home
    10. Pre-visit courtesy call from responsible nurse
    11. Nurse visit * Within 72 hours after application
  • Inpatient Home Nursing Referral
    1. Home nursing referral issued from the ward
    2. Ward physician enters home nursing referral * 3–4 days prior to discharge
    3. Ward nurse contacts the home nursing team
    4. Home nursing consultation: Main Building 5F / Cost + visit area + visit frequency / Referral to partner hospitals or welfare services if needed
    5. Complete consent form
    6. Education on home patient care methods
    7. Purchase and preparation of patient supplies
    8. Deposit payment at administration office * Government-covered patients: No deposit required
    9. Discharge and return home
    10. Pre-visit courtesy call from responsible nurse
    11. Nurse visit * Within 72 hours after application
