Home-based hospice care is a service in which a hospice team—comprising a physician, a specialized nurse, and other professionals—visits the patient’s home, the place where the terminally ill cancer patient and their family wish to stay. The team provides physical, psychological, social, and spiritual care to help patients maintain a high quality of life and live comfortably in familiar surroundings.
Eligible Patients
Terminal cancer patients who wish to receive hospice and palliative care at home.
Application Method
Inpatients
Apply for home hospice through the attending physician or nurse before discharge.
Patients from other departments may apply after a consultation with the Department of Family Medicine (Home Hospice Director).
Outpatients
Apply after an outpatient consultation at the Department of Family Medicine (Home Hospice Director).
Patients from Other Hospitals
Bring a physician’s statement or referral letter confirming terminal status and apply for home hospice after an outpatient consultation with the Department of Family Medicine.
Application Procedure
Apply through the Department of Family Medicine (attending specialist) during an outpatient visit or before discharge.
Complete the Palliative Care Consent Form.
Consultation with a home hospice nurse.
Begin home hospice care.
Home Hospice Cost
5% patient co-payment applies.
No co-payment for persons of distinguished service to the state with full government coverage.
For families of persons of distinguished service to the state, war veterans, and distinguished service medal holders, co-payment applies according to the eligible discount rate.

Visit Frequency
The number of visits may be adjusted depending on hospital scheduling.
