Required Documents for Issuance of Medical Record Copies
Medical records are managed in accordance with the Medical Service Act and the Personal Information Protection Act to protect patients’ privacy and prevent infringement of their personal rights.
Copies cannot be issued without the required documents.
When requesting a copy of medical records, the applicant’s identity must be verified. The patient, family member, or authorized representative must bring the relevant documents listed below.

Acceptable ID Cards: Resident Registration Card, Driver’s License, National Merit Certificate, or Veterans’ Certificate with a photo attached. (Health insurance cards are not valid for identification.)
The Consent Form and Power of Attorney must bear the patient’s handwritten signature.
For patients under 17 years old, identity can be verified using a passport, student ID, or a Family Relations Certificate/Resident Registration (copy or original).

Deceased Patient: Death Certificate, Applicant’s ID Card, Proof of Family Relationship
Unconscious Patient: Medical Certificate of Unconsciousness, Applicant’s ID Card, Proof of Family Relationship
Missing Person: Court Decision of Declaration of Disappearance, Applicant’s ID Card, Proof of Family Relationship
Legally Incompetent Person: Court Decision of Incompetency or Psychiatric Medical Certificate Proving Incompetence, Applicant’s ID Card, Proof of Family Relationship
Military Personnel: Military Service Certificate, Applicant’s ID Card, Proof of Family Relationship
Incarcerated Individual: Certificate of Incarceration, Applicant’s ID Card, Proof of Family Relationship
For deceased patients who passed away at this hospital, a death certificate is not required.
For inpatients who are unconscious, a medical record copy request can be submitted after verification by the attending physician.
Medical Record Copy Issuance Fee
1,000 KRW per page (up to 5 pages), 100 KRW per page for 6 pages or more
