  • Inpatients: Apply for home nursing through your attending physician or nurse before discharge
  • Outpatients: Apply for home nursing after an outpatient consultation in each department
  • Patients from other hospitals: Bring a referral letter or medical certificate from another hospital and apply for home nursing after an outpatient consultation in the relevant department

  • Outpatient Home Nursing Referral Process
    1. Outpatient appointment (+82-2-2225-1234)
    2. Registration at the Administration Office
    3. Consultation, physician interview, and home nursing referral
    4. Home nursing consultation (Main Building 5th floor: cost + visit area + visit frequency, with referral to designated contracted hospitals or welfare services if needed)
    5. Fill out consent form
    6. Education on home care management methods
    7. Purchase and prepare patient supplies
    8. Pay deposit at Administrative Office * No deposit for government-supported patients
    9. Discharge and return home
    10. Preliminary phone call from the assigned nurse
    11. Home visit by assigned nurse * Within 72 hours after application

    2. Ward physician enters the home nursing referral * 3–4 days before discharge
    3. Ward nurse contacts the Home Nursing Team
    4. Home nursing consultation (Main Building 5th floor: cost + visit area + visit frequency, with referral to designated contracted hospitals or welfare services if needed)
    5. Fill out consent form
    6. Education on home care management methods
    7. Purchase and prepare patient supplies
    8. Pay deposit at Administrative Office * No deposit for government-supported patients
    9. Discharge and return home
    10. Preliminary phone call from the assigned nurse
    11. Home visit by assigned nurse * Within 72 hours after application
