When patients are referred to the VHS Medical Center after being diagnosed with or suspected of having serious conditions such as cancer, heart disease, or cerebrovascular disease at another hospital, we provide prompt coordination with the appropriate medical department for timely care.

• If a transfer is needed during hospitalization at another hospital, we also assist with consultation and outpatient appointment scheduling.
• Follow-up management : If a referring medical institution requests information, we provide feedback on the patient’s diagnosis and treatment details after their consultation.
• Re-referral : When necessary, after completing acute-phase treatment, we coordinate the patient’s return to the referring medical institution for continued care.

If a connection to a tertiary hospital is required during outpatient treatment or hospitalization, we assist with prompt outpatient appointments at the appropriate medical institution through on-site consultation at the Medical Collaboration Center after completing the Referral for Medical Care form.

After inpatient treatment for an acute condition, if a patient requires continued care for mild outpatient follow-ups, prescription renewals, or transfer to a local hospital, we assist with smooth coordination to the appropriate medical institution through on-site consultation at the Medical Collaboration Center, following completion of the Referral Back Form.
