Admission and Discharge Procedures

Patients of Following Conditions

Chronic geriatric diseases such as dementia, stroke, cerebrovascular disease, musculoskeletal disorders, cerebral palsy, Parkinson’s disease, pressure ulcers, diabetes, moderate-to-severe pain management, and patients requiring tube feeding

Admission Procedure

  1. 01Registration : Bring the required documents and complete registration at the Administration Office (1F, Building 3)
  2. 02Consultation : Admission consultation with a nurse (1F, Building 3) *Required documents will be checked.
    • Conduct an admission consultation and complete the consultation record.
    • Verify whether the patient is suitable for admission to a long-term care hospital.
  3. 03Admission Approval : A specialist reviews the documents. *Takes approx. 1 week.
    • If approved, please wait for admission (admission proceeds once a room becomes available).
    • If not approved, the reasons will be explained at the Admission Consultation Office.
    • If the waiting period exceeds 1 month, resubmit documents for re-approval.
  4. 04Admission Reservation : Coordinate admission schedule and confirm the reservation.

    Admission preparations, such as COVID-19 PCR testing, will be communicated by phone or text message.

  5. 05Admission Check-In : Admission check-in at the Administration Office (1F, Building 3) *Accompaniment by a guardian is required.
    • Complete documents such as the Admission Agreement and Special Pledge.
    • Submit ID and proof of a negative COVID-19 self-test kit.
    • Attend specialist consultation (1F clinic or ward, Building 3) before admission.

Required Documents

  • Patients admitted from the VHS Medical Center : Long-Term Care Benefit Transfer Form
  • Patients admitted from other hospitals or receiving home care : Long-Term Care Benefit Transfer Form, medication history, and most recent test results within 3 weeks.
    Blood tests [CBC, LFT, BUN/Cr, Electrolytes, CRP, HbA1c (for diabetic patients only)] / urine analysis (U/A) / chest X-ray (Chest PA)
  • The Long-Term Care Benefit Transfer Form can be replaced by a Referral Letter, Medical Opinion, or Medical Certificate

Discharge Procedure

Complete Discharge Procedure

  • Discharge Approval : When the attending physician decides on discharge, the ward nurses’ station will provide guidance on the final medical bill payment procedures.
  • Payment of Discharge Fees : Please make the payment at the admissions/discharge payment counter at the Administration Office.
  • Complete Discharge Procedure : After paying the medical fees, submit the Discharge Completion Certificate to the ward nurses’ station. Then, review your discharge medications and precautions before returning home.
  • Discharges at Night and on Holidays : Regular discharge procedures are available at the Administration Office until 5:30 PM on weekdays. For nighttime or holiday discharges, complete preliminary payment at the Emergency Department, and then settle the final payment at the Administration Office within one week.
