(Lunch time: 12:30 p.m. – 01:30 p.m. / Registration deadline: 04:00 p.m.) - The individual who wishes to fill out the form must visit in person with an ID (such as a resident registration card, driver's license, or veteran’s card) and must receive counseling and an explanation before filling it out.

You can use the elevator to move between the day ward and the operating room. The Advance Directives Counseling Room is located in the center, and next to it are the men's and women's restrooms and the office. Using the elevator on the right allows for additional movement. Below, the surgical ICU, caregiver waiting room, neurosurgical ICU, medical ICU, cardiovascular ICU, and cardiovascular imaging room are arranged in order.
