What is the Advance Directive on Life-Sustaining Treatment?

Anyone aged 19 or older can prepare a document stating their wishes regarding life-sustaining treatment and hospice care in anticipation of becoming a patient in the dying process.
To prepare an advanced directive for medical care, one must visit a registered institution designated by the Ministry of Health and Welfare to receive sufficient explanation and complete the document.
The complete Advance Directive on Life-Sustaining Treatment must be stored in the database of the Life-Sustaining Treatment Information Processing System to be legally recognized.
The VHS Medical Center was designated as an Advance Directive Registration Agency in August 2019 and has been operating as such since then.

  • The individual who wishes to fill out the form must visit in person with an ID (such as a resident registration card, driver’s license, or veteran’s card) and must receive counseling and an explanation before filling it out.
