Hospice and Palliative Care Information

STEP 01Patient diagnosed with a terminal illness (general ward)
STEP 02Confirm patient and family consent for hospice and palliative care services
STEP 03Attending physician submits a consultative hospice referral
STEP 04Complete the Consultative Hospice & Palliative Care Consent Form
STEP 05Begin providing consultative hospice services
STEP 06Service conclusion (discharge, death, or withdrawal of consent)
Referral (to inpatient hospice or home-based hospice)
