Admission Procedure

For Outpatient Visits : Family Medicine & Palliative Care Clinic → Eligibility assessment (by Palliative Care Specialist) → Admission consultation (by an Advanced Practice Nurse) → Admission (Completion of the Hospice and Palliative Care Consent Form and waiting for bed availability required)

While Admitted to the VHS Medical Center: Referral to Hospice and Palliative Care by the attending physician → Eligibility assessment (by Palliative Care Specialist) → Transfer/ward change consultation (with an Advanced Practice Nurse) → Transfer/ward change (Completion of the Hospice and Palliative Care Consent Form and waiting for bed availability required)

Required Documents

If a caregiver (spouse or immediate family member) attends an outpatient consultation, the patient’s prior consent is required for the use of hospice and palliative care services and for decisions regarding life-sustaining treatment withdrawal.

Required documents ※ Please make sure to bring the following documents for the consultation.

Medical opinion or referral letter
- Must clearly state “terminal cancer” or “hospice·and palliative care patient.”
- For in-hospital cases, a consultation request form may be used as a substitute.
Family Relation Certificate (*detailed version)
- Must be issued under the patient’s name (Issuance subject: patient’s name).
- If a family member is attending the consultation on behalf of the patient, please bring two copies.

Additional documents ※ These documents are required by the time of admission.

Copies of medical records
- Physician’s progress notes, blood test results, CT/MRI and other examination results, and a list of current medications
Recent imaging results (CT, MRI, etc.)
A copy of the resident registration
